Non return phone fee

I have talked to numerous AT&T people about this for hours on several occasions we returned the phone but do not have a tracking # did not feel I needed to keep it after 4 month of time We used the shipping label that came with my upgraded phone to return it just as it said to do 

I just want the fee removed for some reason AT&T must have misplaced the phone
