How was it paid for? There should be a line item on your credit card statement or your AT&T bill.

Thank you, I did call att and ask for a receipt. They sent me an invoice but it does not have the device's IMEI or Serial Number on it. Apple needs a receipt with either of those "identifier" numbers on it.
Also, it was paid for by the previous account owner (my father) and he has passed away.

You need an original purchase receipt.
The original receipt has the serial number, not the IMEI. I had to take a quick look back at my records and my original purchase receipt for phones in 2014 does indeed show the make and model of phone with the serial number for each device. Both phones were purchased in a corporate store not online.
I dug up an invoice for a tablet I purchased that was shipped to me, and that shipping invoice does not include IMEI or serial number.
