Trying to submit arbitration paperwork

The form for Arbitration states:
Mail a copy of this form, a copy of the AT&T arbitration provision (available at att.com/disputeresolution),and a
check or money order for the filing fee to the American Arbitration Association Case Management Center nearest
you. Please consult the AAA’s fee schedule for consumer arbitrations for the appropriate fee. Information
regarding the nearest Case Management Center and the appropriate fee is available at adr.org or by calling AAA
Customer Service at800.778.7879.Or, you may file online using AAA Web File:adr.org.

the link above (att.com/disputeresolution) is coded to actually be https://www.att.com/support/article/wireless/KM1041856. This link leads to a page not found message.  Anyone know where the correct link to this apparently necessary page is?

To start the dispute-resolution process under the AT&T arbitration provision, you can:
If after 60 days you’re still not satisfied, you may commence arbitration with the American Arbitration Association (AAA). Here is a sample form you can complete and submit to the AAA: Arbitration Initiation (PDF, 107KB)
