 The first thing the form asks you is if you have "an AT&T mobile phone number" . The default in the form fills in the bubble that says "yesc.

You should change it and fill in the bubble that says "No". 

I chose No and filled in IEMi prompted the server that there was no data.

The website doesn’t work for many users outside of the US. Try using a VPN to a US based network.
