Rural Internet - 5G Cellular router
I have watched videos that show the ability to provide high speed internet through the use of a 5G Cellular Router. when there is no other option for home internet. They don't provide the 300mb to 1000mb service I am used to, but I could get by with 40mb down/15mb up speeds. I currently have 4 ATT wireless lines. Can I add a 5th line and use that SIM card in a 5G Cellular router? Will that work off the unlimited part of the plan? Or is that considered to be the 50mb hotspot part of the plan?

No. You can not add a hotspot device to an unlimited plan.
No. The hotspot device would need its own plan and ATT has no options for hotspot devices on postpaid. You would need to get a prepaid plan for the hotspot device.
