Issues Unlocking AT&T Cingular Flip IV

Have had this prepaid account for a few years.  As part of the 3G phase out they sent a brand new AT&T Cingular Flip IV phone last December. It is eligible for unlocking (over 6 months have passed), and they have confirmed it is eligible when I called them. However, the device unlock site won't work to unlock it - it says the information is incorrect (using phone number), or the other option it says it is active on an account.

When I called again, they said somehow the activation is mismatched with postpaid and they escalated it.  The next day they closed the case with the resolution is to download the Device Unlock app.  There is no download app for the flip phone.  Have not been able to get past that with them and the agent said they don't have access to do anything else.

Is there someone here that could help with this?
