saving images in email - can't "Save image as..." anymore

Until about 2 weeks ago (mid sept 2023) I was able to right click an image from an email and save it by whatever name I wanted.  Now  it auto downloads, saves as IMG # and I have to go to downloads and actually right click there to rename it.   Right click no longer allows me to "save image as....."   Preview sometimes allows me to do this - unless, of course, preview isn't working.  What changed and why? And what do I now need to fix - somewhere - so I can save things the way I used to?  I feel as if I'm back in 2001.

Based on the browser that you use, we'd recommend that you make changes in your download settings i.e. enable "Always ask before download" feature.

I apologize for not having provided feedback yet.  I looked for the settings on my computer and haven't been able to find it and then have been busy. 
I'm not having this problem with google / chrome and pictures sent via my work email and apps.  I work with a tattoo company so I'm dealing with a lot of photos.   

On the other hand, the issue I'm having began after a download app showed up on my Chrome extensions bar, probably via an auto update or something.  So I am perplexed as to why I'm having difficulty with AT&T email but not gmail.

I should have time to mess with this more tomorrow.   Unless you have insight as to why it's on my Chrome bar but not affecting my gmail?

I tried to upload a screenshot of the section on my taskbar, but maybe this only uploads jpegs
