It is possible to use IP Passthrough and Cascaded Router simultaneously if:
You have a Public Static Block of IPs that you want controlled by the very same router that you also want to handle unsolicited traffic for the Public Dynamic IP address. That is an advanced configuration for most routers, so it should not be undertaken lightly.

I do not know if that's possible, but you can certainly test it yourself.
There's another group of users over at DSLReports that might have experience with that.

IP Passthrough is intended to work with the Public Dynamic IP address.
Cascaded Router is a configuration used with a Public Static Block (which is an additional cost option).
You use the one that relates to what you want to do.
