The permission to unlock your phone comes from the carrier, but the actual unlock Hass to be completed by connecting with Apple servers. Either Wi-Fi or a cellular connection should complete the unlock process.
Apple can take up to 48 hours to complete the unlock process.
If it has already been two days since your approval and the phone is still not functioning with another Network, contact Apple and find out if your IMEI was submitted for carrier unlock. If they have no record of an unlock request then you have to resubmit through AT&T. Either AT&T or Apple requested the wrong IMEI

I called Apple support, they see the device as locked. AT&T has sent approval twice. I'm confused

If Apple still shows the device as locked, then it’s very likely that something went wrong with the AT&T request. The only solution is that AT&T resubmit the request. Convincing them to do it is going to be the hard part when they show the request already approved. May be best to file a FCC complaint to save going back and forth with Customer Service.

We want to help you get your phone unlocked, @ShinAL.
We'd like a little more information to narrow down what's going on with your unlock. What happens when you insert the other operator's SIM card into the device, does it give you any error or does it not pull up the unlock code screen? What type of SIM card are you using? If the other operator uses the AT&T network, then you won't be able to enter the unlock code and complete the unlock process.
Our ACEs also gave great information as well. If 2 days have already passed since the unlock request was submitted, try submitting it again. You can also check the status of your IMEI to see if it's consider locked or unlocked.
Please share the results with us here.

When I insert the SIM card of another operator, the phone goes into activation mode and gives an error when trying to activate. The screen says "the SIM card is invalid, insert a SIM card belonging to a supported operator or request an Iphone unlock." I use Nano-sim. The unblocking status is approved.

@Gongziyi The network message usually means that you’re trying to access the site from a foreign ISP that is blocked. Try using a US-based VPN.

Thank you for sharing more details about the error you're experiencing while attempting to connect your recently unlocked device to another service providers network, ShinAL!
Let's go ahead and meet in a direct message, so we can confirm if your device has successfully been unlocked. We also like to escalate a device unlock case for further investigation on your behalf.
Make sure you have the IMEI for your device noted and ready to share once we're in a direct message.
You will find our direct message located in the upper right corner of the Forums which is the chat icon next to the bell icon. We look forward to helping you!
Thank you for connecting with the AT&T Community!
