Failure to easily obtain prepaid phone bill info

I need my phone bill for my pre-paid cell phone that clearly states my name, address, #, today's date and the AT&T logo all in one. It is easier for post paid users to get this information, why is it so hard to get mine? I have been a loyal customer for almost 5 years now and all I simply need is a phone bill that I can use to prove that I am the owner/user of my number in order to set up my phone to accept calls from one of my best pal who is incarcerated. I know this information could be found online but the problem is that everything I need is not all together, but on a different section from one another and the TDCJ will not accept any phone bills that are not together. TDCJ recently started to accept prepaid phones, this would really help me out if someone could please just allow us to easily get this bill.
